• Waist, Bra measurements.
• Procedure Interested in and please tell us about your concerns, previous pregnancies and/or medical issues, procedures.
• Attach photos of yourself.
o Use good lighting and a solid background.
o Please take full body pictures, using a selfie stick or taken by another person.
o Include photos from the front and each side and back.
o Upload your photos as JPG files (.jpg)
o Photo maximum size is 2 megabytes (MB)
o Should be at least 1 photo of each desired surgical area. Front/back/left side/right side photos of the area are needed.
• Also, please just state that you are consenting to the virtual consult in your return email and the best contact number for you.

Have you had any previous procedures/surgeries? If so how long ago?
How many pregnancies?
Have you have a C-section or vaginal delivery?
Please know that the doctor's ability to give a recommendation may be limited based on the photos submitted. Please take photos in a well- lit area and it is best if you can have someone else to take the photos for you.

Please allow 5 to 7 days for a response so that Dr. Lissa Bootstaylor can thoroughly review your information and photos prior to returning your email. If you do not receive a reply after 7 days, call our office at 404-240-2804.
